A Questionnaire can be configured to provide a range of Standard Observations you can use in response to each Question. You can select these items as follows:
Procedure
1.On the Question Answer page, click the Standard Text button in the Observations section.
2.It will show the Observations dialog with the standard text available for that question.
3.Click Override or Append against any of the items in the list. The Override function will replace existing text with the selected standard text; the Append function will add the selected standard text to existing text.

Auto Standard Text
A response item for a question can be set up to insert a Standard Observation for you. If the Questionnaire has been configured in this way, you will find that text automatically appears in the Observation fields when that response item is selected and disappears from the Observations fields when that response item is deselected or cleared.
Permissions
The Standard Text button is visible only if related permissions are turned on for the User.
